Summary Statement

The Division uses a standardized process for reviewing serious incidents involving workers.

Purpose

The primary purpose of the Worker Serious Incident Review Team (WSIRT) is to review serious incidents to determine if incidents could have been prevented and what lessons can be learned to reduce future incidents.

Additionally, the WSIRT will utilize the review process as a quality assurance tool to determine if policies were followed, identify areas of non-compliance and identify opportunities for improvement in DAS programs. The WSIRT process is not intended to replace review processes within each program.

It is further recognized that a careful examination of incidents provides the opportunity to develop training, prevention, and strategies that will lead to improved worker safety.

Basic Considerations

WSIRT Members

The WSIRT shall include the following members:

  • WSIRT Facilitator (Senior Field Operations Manager)

  • Public Guardianship Office (PGO) Program Owner or designee

  • Adult Protective Services (APS) Program Owner or designee

  • Livable Communities (LC) Section Manager or designee

The WSIRT facilitator, DAS Director, Deputy Director(s) or Senior Field Operations Manager may assign additional DAS program designees and/or other third parties as needed.

Scope

Worker Incidents resulting in

  • A Workplace Acts or Threatened Acts of Violence Report

  • A Worker’s Compensation Report

  • A report to Law Enforcement

  • Any other incident resulting in an Incident Investigation as described in Section 4040 of this manual

The WSIRT will not include incidents at the contractor or provider level.

The WSIRT will meet no less than quarterly to review all incidents in the DAS Data System (DDS) worker records.

The WSIRT Facilitator will obtain the DDS report for the previous quarter’s incidents and make a preliminary determination of trends and/or categories of incidents and draft an agenda based on those findings.

The WSIRT Facilitator may request supporting reports (EAR, Workplace Acts or Threatened Acts of Violence, Workers' Compensation, etc.) from supervisors in preparation for the meeting.

The WSIRT will review the past quarter’s reports, determine trends, make recommendations at the following levels:

  • Division wide policy/procedures

  • Department level recommendations

  • Program/section policy/procedures

Program Owners and Section Managers are expected to act upon the recommendations of the WSIRT.

Reports

The WSIRT Facilitator or designee shall issue a report to DAS Management Team no later than 30 days after each WSIRT meeting to include:

  • Number of incidents in the past quarter

  • Recommendations

After review by Management Team, the report shall be made available to all DAS staff via email from the Director’s Office.
