Summary Statement

The Division of Aging Services uses Measurement and Analysis Plans) to ensure continuity and consistency in data reporting.

Basic Considerations

The Division continuously monitors its performance on key measures and/or programmatic operational measures through standard reports and dashboards. Measures may be tracked daily, weekly, monthly, quarterly, or annually. Each measure is related to a specific goal or objective, and measures are used to identify gaps in service delivery, drive solution-oriented innovations, and ensure continuous quality improvement.

Performance measures or the staff managing and monitoring the data may change over time. MAPS manages the knowledge of performance measures, ensuring that both historical and current information are transferred seamlessly to new staff.

Procedures

Program staff are required to submit a MAPS Form for each new performance measure identified. Additionally, MAPS forms are updated quarterly by program staff to provide an ongoing data trend, along with a narrative explanation of any anomalies or deviations from expected performance.

MAPS forms are stored in the DAS WIG Team SharePoint.
